We truly do live in exciting times. People are living longer and healthier but with this increased longevity is the increased development of chronic medical conditions. These condition include things like diabetes, heart disease, high blood pressure, and many others.

WHATS THE BIG DEAL ABOUT CHRONIC DISEASE?

The Centers for Disease Control (CDC) demonstrated that 45% of adults over the age of 65 have 2 or more chronic conditions. These conditions can have serious consequences and cause you to be “sicker” more often. Some of the conditions can even predispose you to developing other diseases. However, there is hope. With proper medical care and monitoring, we are able to prevent many of these other conditions, slow the progression of the disease and the burden that it brings. That’s why we want to take our care a step further and assemble a team for your healthcare.

No matter who your physician currently is, you are most likely receiving care of some chronic medical condition that requires your doctor to schedule regular visits with you. Healthcare reform has taken us into a national debate over how we should proceed forward in the care of our people. However, no matter which side of the debate you stand on, most people agree that primary care and preventative medicine should become more valued. It is easier to prevent a problem from developing than it is to fix its damage once it has occurred. Insurers, both private and public, have begun to recognize medical groups who excel in their management of chronic disease. We wish to be the absolute best provider of chronic disease management in Citrus County.

Doctors are a large part of healthcare, but they are not the only team members that provide care for patients. Nurses, pharmacists, and other individuals play an integral part in the care we provide. We decided we cannot provide the level of excellence we desire without bringing some more people on board. We have also partnered with i2i Systems makers of an innovative population health software to help design software that ensures every patient is receiving the same optimal level of care in our population.

WHAT DOES  THIS MEAN FOR ME?

Beginning in 2015, you will begin to hear from us a little more frequently. We are optimizing our medical records to make sure that you receive the most evidence-based medicine and optimal care for all of your conditions. We may call occasionally to check in on you or to determine if you need to visit our office so we can prevent you from developing complications from any of your conditions. We have a team available 24/7 coordinating your care within all aspects of our healthcare community.
